I still agree with the name, it can slide while you're relatively unknown, but if you start getting popular? Holy hell, will you feel the fury. And that's regardless of your explanation in the description, keyboard warriors shoot first and ask questions later, not many are willing to listen to what you have to say before crucifying you. Also gamers are pretty well known at this point for taking things out of context, so y'know, use your name at your own discretion.
I also agree you could try experimenting with shorter vids. Though gameplay videos are not my domain, I think people are generally less likely to sit through a 3 hour video, much less subscribe for more of them unless every second of it is vital to the video. As for focusing on Fallout, I'm not sure you should do that unless you really want to be a Fallout-centric channel. You're at the stage where you're establishing your channel's identity, if people end up subbing for Fallout, they'll expect Fallout and are likely to lose interest if you stray too far from it.
Another thing to consider for raising subs is always, and I don't know how active you are with this, promotion outside of youtube. Think forums, reddit, Steam and the like. Just becoming a part of a community and occasionally mentioning you've made a video to people who might be interested (in a respectful, non-spammy way) can give you the bump you're looking for.
