Privacy Complaint Form Won't Work..?

I found out there was a video of me on YouTube and I have tried getting it taken down via the privacy complaint form but every time I fill it out and click submit it tells me: "There were problems sending your form. Please try again". I have tried contacting the user that uploaded it but it seems the account is dead. I don't know this person either so I can't ask them via Facebook because I don't know their full name. This video is affecting my work and almost got me fired today, so any help would be much appreciated :)

I've tried using the Youtube complaint form about 20 times and it won't work, I've emailed them and heard nothing back. I know you guys can't help me take it down but I thought other people might have had the same problem and would know what to do. I'm all out of ideas tbh :(

You could try the report function again using a different browser--some browsers can be finnicky. Also, have you tried calling them? There is a phone number listed on the bottom, though I'm not sure what type of call-line it is.
