I have a video that is doing really well that is a tutorial. Can I remake the same tutorial with new footage, upload it, and give it the same title as the other one, as well as the same thumbnail? Is this against any rule? I'm trying to replicate the success.

If it's yours then you have the rights to it. I've re-uploaded a couple videos already with changes. My top two videos were combined to be a split screen view, I was hoping the SEO would work the same with it and put it at the top, but so far no such luck. That one has about 14k views so far. I also added music to one of my original dog videos, they are both still online.
 
I think you must be careful with duplicated images and videos. The algorithm may decide that there is something fishy and suppress (bury down in the search results) one of the videos. I would rather go for a variation of the first one - something like "...Part II" or "... tutorial upgraded" or "... the better version" and so on.
Try to be more creative and let the algorithm decide which one of your videos to promote. Also keep in mind that now you start to compete with yourself (if both tutorials are more or less the same).

why all videos I upload going direct to "[!] provide proof" and after I provided the proofs never monetize it? (it always show [?] under review) this situation have 90 days also.


How can I fix this in my account?

anothers videos before 2014 has been monetized normal.
