YouTube partnership Networks?

Okay guys hi :) I'm new here :P
Uhm, so the people that are partners with a 3rd party Network Witch network did you partner with and did you have to fill out a W-9 Form?, B/c Makers studios Accepted me But i couldn't Complete the process b/c i could not fill out the W-9 Form,it would be awesome if you could point me to some Networks. Thanks

Why trouble with filling out W-9 form?

You don't have to be 18 to fill it out. I know people who are younger and filled it out. Don't live on their own, don't have a job, and you don't need to pay taxes.
