Associating an Adsense account

Do i need to just enable monetization or also fill this form? imgur.com/HR6F2Rp
I have this pending form since i became YT partner but i didnt filled it because i just needed the thumbnail.
I havent got an answer from them yet so maybe someone from here knows.

Your question doesn't make 100% sense, but I'll try and answer it...

If you've been accepted into a network you need an active AdSense account. Enabling monetisation won't make that happen, you need to enable AdSense properly (as in provide a place for them to pay money to, fill out your details etc).

Go to Google AdSense and set one up.
 
In their "tutorial" that form is not mentioned. Also they already got my details from the other forms and also the contract and they pay via PayPal so i dont know.. Maybe someone who got recently partened with Machinima is around?
 
I heard having an AdSense associated makes the partnership process much longer since they have to disable it again...?

EDIT: Also, if they pay via PayPal, why would you need Adsense?

They pay via PayPal but you still need monetization enabled which is linked to AdSense somehow.
I still need an answer if i should complete that AdSense form or not.
 
You need to create a Adsense account in order to approve your Monetization. It has nothing to do with paying you or whatever, Networks can't link channels with Monetization enabled but no Adsense.

I did created an account but just havent filled that form. You sure i need to fill it? I currently dont have an credit card so that might take even longer :/

Fill it in then. Adsense don't pay you until you reach $100 USD so not sure what you mean with the credit card.. they pay you by Bank transfer or cheque (check) anyway

This form (imgur.com/HR6F2Rp). Says "My Payee need to math the name on my bank account"
